Job Summary
We're currently seeking an experienced Registered Nurse - Case Management to join our team on a travel assignment. As a key member of our case management team, you'll play a vital role in coordinating patient care across multiple settings and services. Key Responsibilities
- Patient Care Coordination: Collaborate with healthcare professionals to develop and implement individualized care plans that address the unique needs of each patient.
- Care Plan Development: Conduct thorough assessments of patient needs and create tailored care plans that prioritize patient-centered care.
- Interdisciplinary Collaboration: Work closely with healthcare professionals, including physicians, therapists, and other nurses, to ensure comprehensive patient care.
- Patient Progress Monitoring: Continuously monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Patient and Family Education: Provide patients and their families with education and resources to support their care plans and promote healthy behaviors.
- Documentation and Reporting: Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements and organizational policies.
Requirements and Qualifications
Essential Qualifications:
- RN Licensure: Current RN licensure in the state of Oklahoma or eligible for licensure.
- Experience: At least 2 years of experience in case management or a related field.
- BLS Certification: Basic Life Support (BLS) certification.
- National and State RN Certifications: National and state RN certifications as required.
Preferred Qualifications:
- Case Management Certification: Certification in case management, such as CCM or ACM.
- Experience with Electronic Health Records: Proficiency with electronic health records (EHRs) and other healthcare software.
