Key Responsibilities
- Lead and collaborate with our care team to develop and implement comprehensive patient care plans.
- Provide high-quality, patient-centered care to individuals in Butler and surrounding areas.
- Utilize your clinical expertise to assess patient needs, develop targeted interventions, and evaluate outcomes.
- Work effectively in a fast-paced, dynamic environment, prioritizing tasks and managing your time efficiently.
- Maintain accurate and timely documentation, meeting critical deadlines for charting and other administrative tasks.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Participate in ongoing education and professional development to stay current with best practices and industry advancements.
Essential Qualifications
To succeed in this role, you'll need:
- A current RN license in the state where you practice.
- Graduation from an approved school of practical nursing.
- A minimum of one year of RN experience, with demonstrated clinical competence in delivering quality patient care.
- A valid driver's license and the ability to spend approximately 20% of your day driving to and from patient locations.
- Proficiency in using tablets, mobile phones, and EMR software.
- Strong time management and organizational skills, with the ability to self-manage your schedule.
Preferred Qualifications
While not required, the following qualifications are highly valued:
- Experience in home health or hospice care.
- Knowledge of industry best practices and current healthcare trends.
- Certifications relevant to home health or hospice care, such as OCN or COS-C.
- A degree or certification in a relevant field, such as a BSN or MSN.

Compensation and Benefits
