Key Responsibilities:
- Deliver expert health education and coaching to members, enabling them to navigate the healthcare system with confidence
- Facilitate seamless transitions of care by coordinating services, referrals, and appointments
- Conduct thorough assessments to identify gaps in care and develop targeted interventions to address them
- Provide empathetic support and guidance to members, addressing their unique needs and concerns
- Cultivate strong relationships with members, providers, and internal stakeholders to drive positive health outcomes
- Utilize advanced clinical skills, critical thinking, and problem-solving to address complex member needs
Requirements:
- Current, unrestricted RN licensure
- 3+ years of clinical RN experience, with a strong foundation in clinical skills and critical thinking
- Proven expertise in telephonic communication, with excellent inbound and outbound skills
- Intermediate computer skills, including Microsoft Word, Outlook, and Internet navigation
- Ability to work in a fast-paced, dynamic environment, with a focus on delivering exceptional member experiences
Preferred Qualifications:
- Bachelor's degree in Nursing
- Coaching and/or decision support experience
- Managed care experience
- Conflict management skills
- Proven understanding of change management principles and the ability to motivate members towards positive health behaviors
