Position Overview CloudCath is seeking a Qualified Healthcare Professional (QHP) to support our Remote Patient Monitoring (RPM) program for patients receiving peritoneal dialysis (PD). This role involves the clinical review and interpretation of device-generated patient data, identification of potential early indicators of peritonitis risk, and collaboration with prescribing nephrologists and dialysis clinic teams to support timely clinical intervention. The QHP will perform monthly clinical data review and patient management activities required for CMS RPM services, ensuring all services are documented in accordance with CMS guidelines and applicable payer requirements. This position is ideal for a Nurse Practitioner (NP) or Physician Assistant (PA) with experience in nephrology, dialysis, or remote patient monitoring who is comfortable working independently in a remote environment. Key Responsibilities • Conduct monthly clinical review and interpretation of CloudCath monitoring data for assigned patients. • Assess turbidity and device trend data to identify potential early indicators of peritonitis risk or other clinical concerns. • Document clinical interpretation, time spent, and management activities in accordance with CMS RPM documentation requirements. • Communicate clinically relevant findings or alerts to the prescribing nephrologist and/or dialysis clinic care team. • Participate in established clinical escalation workflows when abnormal trends or alerts are identified. • Maintain accurate and timely documentation within the CloudCath RPM platform and associated clinical systems. • Support ongoing compliance with CMS regulations, payer requirements, and clinical best practices related to RPM services. • Collaborate with CloudCath clinical leadership to continuously improve monitoring workflows and patient care processes. Qualifications Required • Licensed Nurse Practitioner (NP) or Physician Assistant (PA). • Active, unrestricted Nevada license required. • Ability to provide clinical services remotely in compliance with applicable regulations. • Strong clinical assessment and documentation skills. • Ability to work independently and manage assigned patient monitoring responsibilities. Preferred • Active California license. • Clinical experience in nephrology, dialysis, or peritoneal dialysis. • Experience with remote patient monitoring (RPM) programs or digital health platforms. • Familiarity with CMS RPM billing and documentation requirements Work Schedule • Part-time, remote position with flexible scheduling. • Monthly workload will vary based on assigned patient caseload.
