For more than 20 years, we’ve provided high‑quality, home‑based primary care to older adults who are homebound, medically complex, or living with chronic conditions. As part of the Parker Advanced Care Institute at VNA Health Group, our interdisciplinary team — physicians, nurse practitioners, and physician assistants — delivers comprehensive in‑home medical care, from routine visits to chronic disease management, telehealth, diagnostics, and wound care. Position Summary:  The Physician is responsible for providing high‑quality, compassionate primary care to homebound patients, ensuring their symptoms are well‑managed and their quality of life is maximized. This role offers the opportunity to build meaningful patient relationships while helping individuals remain safe, stable, and well‑supported in the place they call home. Principal Responsibilities: Patient Care & Clinical Impact - Provide direct medical care and consultation to homebound primary care patients within established productivity guidelines - Monitor patient conditions and adjust treatment plans based on clinical needs and responsiveness - Serve as a compassionate, professional representative of the agency to patients, families, and community partners Collaboration & Team Contribution - Collaborate with colleagues across the interdisciplinary team - Contribute to both formal and informal staff education - Participate in community outreach efforts to promote Visiting P services Documentation & Compliance - Maintain accurate, timely documentation in the Electronic Medical Record - Ensure compliance with NJ Department of Health regulations and all relevant licensing and accrediting requirements - Adhere to all infection control and safety policies Mission & Culture - Uphold the organization’s mission through excellence, professionalism, and positive teamwork Job Competencies & Minimum Qualifications: - Licensed as a physician in New Jersey with education and experience in primary care settings - Geriatric experience recommended - Board certified in family practice or internal medicine - Active CDS and DEA license - Excellent verbal and written communication skills - Excellent organizational and interpersonal skills - Car and valid NJ driver’s license Why Join Us - Make a meaningful impact by caring for a vulnerable, deeply appreciative patient population - Build stronger, more personal patient relationships than in traditional office‑based medicine - Work within a supportive, collaborative interdisciplinary team that values shared expertise - Practice medicine with autonomy, purpose, and a focus on whole‑person care Working Conditions/Physical Demand: Traveling to patients’ homes.  Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing, walking, lifting 40 pounds, pushing, reaching, pulling #LI-AD1 - In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $200,000.00  to USD $296,000.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
