Centers Health Care is seeking compassionate and experienced Licensed Practical Nurses (LPNs) to join our team on a part-time basis at our Skilled Nursing Facility in Brightwaters, NY. As an LPN at Centers Health Care, you will play a vital role in delivering high-quality patient care and supporting our team of healthcare professionals.

About the Role:

• Collect and record patient information, including medical records and insurance details
• Monitor and record patient vital signs, including temperature and other health metrics
• Administer medications and injections as prescribed by physicians
• Provide wound care and dressing changes as needed
• Supervise and support Certified Nursing Assistants (CNAs) as required
• Assist patients with daily living activities, including dressing and personal hygiene
• Monitor patient food and liquid intake and output

Requirements:

• Valid state LPN license
• Strong teamwork and communication skills
• In good standing with the State Registry
